چکیده

IT SEEMS likely from human and animal studies (Kincaid-Smith, 1967; Abrahams & Levin, 1967; Kincaid-Smith, Saker & McKenzie, 1968) that kidney damage resulting from a high analgesic intake in man and in animals can be explained on the basis of papillary necrosis followed by subsequent atrophy of adjacent cortical tissue. This concept makes it much easier to understand the possible mechanisms whereby analgesiccompounds may damage the kidney. Many substances are present in very high concentrations in the papilla, and it is reasonable to postulate that damage to the papilla may be related to high concentrations of some breakdown products of analgesic drugs. This paper discusses possible mechanisms which may result in papillary necrosis in different conditions, with special reference to papillary necrosis associated with a high intake of various analgesic compounds.

Analgesic nephropathy in the United Kingdom: Incidence, clinical features and pathogenesis.

Phenacetin came into use in 1887, just 10 years after the first case of renal papillary necrosis was reportedl 2 but the association between the excessive use of phenacetin-containing analgesics and the development of renal damage was not recognised until 1953,3 since when there has been gradual recognition that excessive analgesic consumption is a major cause of chronic renal disease in many p...

Over-the-counter analgesic use.

Chronic analgesic nephropathy, particularly chronic interstitial nephritis and renal papillary necrosis, results from daily use for many years of mixtures containing at least two analgesics and caffeine or dependence-inducing drugs. Computed tomography scan can accurately diagnose this disease even in the absence of reliable information on previous analgesic use. The occasion to moderate regula...

Analgesic abuse and renal failure in Australasia.

Analgesic-induced renal papillary necrosis first became a significant factor in mortality tables for two Australian states, Queensland and New South Wales, about the year 1960 [1, 21, and the earliest descriptions of the clinical syndrome and renal pathology in Australasia (i.e., Australia and New Zealand) appeared during the following two years [3—5]. Nearly two decades earlier there had been ...
